How to make an Appointment for a Dependent TRC:
New Process in Krakow:
We would like to inform you that – due to the planned launch of the new version of the MOS portal (Case Handling Module) on April 27, 2026 – the currently functioning version of the MOS portal will be deactivated on April 17, 2026.
We would like to inform you that from April 27, 2026, the MOS V2 system (Case Management Module) will be launched - a portal for submitting applications for residence permits in Poland in electronic form.
From the indicated date, applications for permission for:
temporary stay,
permanent residence,
long-term EU resident stay
will be accepted only in electronic form via the MOS V2 system.
The option of submitting the above-mentioned applications in paper form is abolished.
All appointments scheduled via email from April 27, 2026 onwards are cancelled.
Booking visits via the following address is suspended: rezerwacja.opt@muw.pl (except for matters relating to the replacement of residence cards).
Applications submitted in paper form must be received by the office no later than April 26, 2026. If the application is received after this deadline, it will not be considered (the application will be left without consideration) – the date of mailing will not be taken into account.
Please log in to this portal, complete the application, and then print it.
Once you receive your appointment date, make sure to visit the Urzad office with all your required documents and your application.
1. To schedule your appointment, log into the MOS portal, fill out the application, print it, sign it, and then send it by registered post.
2. After about 15 days, check with the Urzad office to find out your appointment date.
3. When your appointment day arrives, you'll complete your biometric check and get your passport stamped. We're here to help you through every step of this process!

CONNECTING WITH THE FAMILY
Required documents:
This PDF provides information on the necessary documents required to obtain a temporary residence permit in Poland for the purpose of family reunification. APPLICATION
From 2025 onwards, online application filling has started. The application has to be filled in Polish only.

In case the TRC/Blue card is lost, but you have to renew your card, in such use cases, the two forms below have to be submitted.
Justification letter in Polish and English:

1. Application for a residence card (fill in Polish and make necessary changes to the attached - use a translator, create a copy for the second child)
2. ZUS RMUA Last three months
3. ZUS ZUA insurance confirmation - the contract concluded with the National Health Fund and confirmation of payment of ZUS contributions for the last month
A document confirming that a ZUA PZUA family member has been registered with ZUS.
In order to process the case as quickly as possible, the documents should confirm the fact of having insurance for at least a period of time 6 months following the date of submission, in the case of reporting to the Social Insurance Institution - a current document.

4. Rental agreement Original in Polish + copies --- Document confirming the place of residence, e.g.:- a lease agreement for residential premises.
5. Original and Translated Marriage\birth certificate + copies
6. Work Permit and Employment contract in Polish original + copies

8. PESEL / address registration + copies - Address Registration is required at the time of Physical card receiving.

10. Statement of Maintenance for dependents (Statement about covering the costs - Family members-Cover Letter)
Financial resources for the stay, i.e.:
- a valid document confirming that you have sufficient funds to cover the costs of your stay in Poland (at least PLN 600 for each month of stay + 2500 PLN for a return flight ticket), e.g.:
• contract confirming the receipt of remuneration,
• statement of a family member on covering the cost of living + a document confirming the amount of income of this person,
documents should not be older than 1 month from the date of submission of the application
Bank statements with sufficient funds.
Once all the above documents are ready, take all the documents in a file to the below address for verification ( open between 10:00 to 16:00) - they will do this free of cost. Go at least a day before so that if there are corrections in the application form, you can do it well in advance. The address of this office is;

7. How to check case status
The status of the case can be checked through the Information and Hotline (Info.opt) operating in the Małopolska Voivodship Office in the Department for Foreigners. The hotline provides information in the form of:
● e-mail ( info.opt@muw.pl ),
● by phone (12 210 20 20) and
● directly at the seat of the Office (ul. Przy Rondzie 6).
After your biometrics are done on the that further communication be via emailappointment date, after one week,

You will receive your case ID and case officer email ID. Now send an email to your case office to request further communication will be on emails.

The TRC / EU Blue Card case officers are asking for Wet and Digital Signature documents (both), especially Job Contract documents. They want the document's Start and end dates (if your contract is uninterrupted, mention the same). In a simple word, “Employment Contract Certificate” with the latest date.
So, when applying for a TRC / EU Blue Card or renewing, first prepare this certificate (both wet and digital).
When they ask for a Wet Signature certificate, Go physically to the Urzad office (Monday till 6 PM, other days till 4 PM).
When they ask for digital signature certificates, send them through an email.
We have to carry Originals and xerox copies. Original will be verified and give them back to us.
Optional: Employment contract wet signature and digital signature both keep it ready.
Appointment: In Krakow Blue Card or TRC appointment for Biometrics and application submission with original documents verifying, email-based appointment required. I heard that other Cities have call-based and web-based appointments. In 2025 we have to send this email 5 months before.
One the application submission day, Urzad office will stamp on our Passport. With this stamp, we can stay in Poland, even our Visa expired. But can’t cross the Poland Boarders. If you have any emergency to go to India, you can go. But while coming to Poland you need a valid Visa.
Once we give our biometrics and application submission in Urzad office, we need to wait for the decision letter. In between if any queries from Urzad office we have to clear those.
Once the decision letter comes, again we need to wait for the physical card.
Ideally, we can’t change our address and employment in this process period.

To obtain a Temporary Resident card, four passport-sized photographs are required. The Photoshop studio is located near the Urzad office, behind Millennium Bank. The website indicates that they offer a package of eight photographs for 40 Zloty. They will take your photographs and deliver them within five minutes. If a digital copy is available, eight photographs can be acquired for 30 Zloty.

In India, a husband and wife have different surnames on their Passports. When applying for a Polish Dependent Visa for my wife, what is the process?
Answer:
It is completely acceptable for a husband and wife to have different surnames on their passports when applying for a Polish Dependent Visa (National Visa D-type). In Poland, it is common for spouses to keep their respective surnames.
Her passport has her Maiden Name. The Marriage Certificate lists her Maiden Name (or lists her Maiden Name as the "name before marriage").
Question-3:
While submitting your TRC or Blue card application, and after biometrics submission, what are the next steps?
1. After your application and biometrics submission, it will first go to Border Security verification. It takes 1 to 2 months. Once it's done, only your file will reach the case officer. Then you will get the case number and the case officer's email ID.
2. After 1 month of application and biometrics submission, go to the Urzad office help desk, ask for your application Case number, case officer name and email ID. Send an email to your case officer, and you will receive updates from them then on. Otherwise, they will send postal communication, which will be delayed.
3. ZUS - after 3 months of your application and biometrics submission, you need to provide
a) the latest ZUS certificates.
b) Last TRC Decision Letter (If it's a renewal)

4. Every original and Xerox must be equal. For example, if you take ZUS digital certificates, print, and wet-sign the original, they won't accept it.
5. Another Example, any certificate, the back side, if any stamps are there, and you didn't take the Xerox on the backside, they won't take it. Every original and Xerox must be the same.
TRC/ Blue Card Decision Letter Appointment

If your BlueCard/TRC application is underway and you subsequently decide to relocate back to India, it is recommended to submit a formal letter requesting the cancellation of the application. Failure to do so may result in a negative decision at the time of decision-making. Therefore, it is advisable to proactively request the cancellation of the application rather than risk receiving a negative outcome. Such a request can be submitted via email to the Inspector. However, the letter must contain your electronic signature.
I prepared all the documents below (It took one month), and then sent an email to book a TRC slot in May. I got the slot after 3 months..
So, first booking a slot and then preparing the documents is better.
